OUR IMPACT

Through education, advocacy, and community partnerships, the HEM Foundation is working to improve the lives of individuals affected by sickle cell disease and other chronic conditions.

Our initiatives include:

  • Hosting our annual THRIVE event bringing together providers, advocates, and patients

  • Awarding educational scholarships to individuals living with sickle cell disease

  • Providing educational programs and resources for patients and families

  • Expanding conversations around mental health, wellness, and holistic care

Your support helps us continue building a future where every patient has access to equitable care.

Picture of Maya Bloomberg, hematology nurse practitioner and founder/president of the HEM Foundation

Maya Bloomberg is a nurse practitioner, healer, and advocate devoted to transforming the way we care for marginalized communities—mind, body, and soul.

Maya is a board-certified Family Nurse Practitioner and respected leader in hematology and health equity. She has worked as a Classical Hematology Nurse Practitioner since 2014, specializing in sickle cell disease and congenital bleeding disorders, which shed light on the significant health and racial disparities that exist in modern medicine

Trained in conventional medicine and deeply rooted in holistic healing, Maya bridges two worlds: science and spirit, data and wisdom, evidence and energy.

After over a decade caring for individuals with sickle cell disease and bleeding disorders, she saw how many were left behind by systems not designed for their complexity—or their humanity. In response, she founded the HEM (Health Equity Matters®) Foundation to offer a new path forward—one that centers culturally responsive education, mental wellness, and community empowerment.

Through the Foundation, her podcast, and her presence as @theHemeNP, Maya guides others not only through illness—but toward possibility.

Her mission is clear: to create space where healing feels safe, whole, and real—for all.
